Referring Doctors

Referral forms are available from the Idaho Center for Oral and Facial Surgery. These are provided in packets of 50 referral sheets per pad. Please contact us if you are running low on your referral sheets and we will provide you with a new packet. Thank you for allowing us to participate in the care of your patients.

Online Referral Form

You may refer patients to our office by filling out our secure online Referral Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of patient data is one of our primary concerns and we have taken every precaution to protect it.
